Ophthalmoplasty is a part of plastic surgery which aim is reconstruction of anatomic and functional disorders in eye and its appendage. Modern interpretation of the word “plastic surgery” unites surgical methods for restoration of form and functions of different parts of the body or anatomical structures by translocating, transplanting, and implanting substitutional materials.

At present time it is almost impossible to draw a line between plastic and esthetic ocular surgery for they fill each other up in many ways. Normal functions of many anatomic parts of the eye depend on their certain shape.

PLASTIC AND RECONSTRUCTIVE EYELID SURGERY

1. Posttraumatic operations

Our clinic performs many various reconstructive surgeries in case of mechanical injuries, burns, consequences of concomitant injury, damages of orbit, appendage, lacrimal passages, posttraumatic ptosis of upper and lower eyelids, posttraumatic deformations of eyelids and orbit.

2. Surgery for entropion and ectropion of different genesis

Entropion – change in eyelid position when its costal margin is turned inside and lashes touch the eyeball surface damaging it. Entropion happens to the lower eyelid mostly

  • Congenital (associated with marginal layer of orbicularis muscle hypertrophy)
  • Acquired (atonic or age-related, spastic, cicatrical)

Ectropion - change in one or both eyelid position when costal margin is turned outside the eyeball surface and as a consequence the mucosa is turned inside out. Mostly happens with lower eyelid.

  • Congenital (associated with shortening of outer musculocutaneous eyelid plate and usually goes along with some other congenital pathology)
  • Acquired (atonic, age-related, spastic, paralytic, cicatrical, mechanical)

Surgical method of treatment is chosen by the surgeon, depending on mechanism and entropion or ectropion degree. It is important to remember that apart from a cosmetic defect, ectropion or entropion may cause secondary changes in cornea, eyelids and mucous membranes. In some cases, acute corneal inflammations (keratitis) may lead to a corneal ulcer, often with the threat of perforation. Therefore, timely visit to a doctor and properly prescribed treatment are the best prevention for serious complications.

3. Surgical treatment of lagophthalmos

Lagophthalmos is an ophthalmological disorder when eyes do not close fully (eyelids do not close completely therefore occurs cornea inflammation) when facial nerve is paralyzed or there is some congenital eyelid pathology, or eyelids have cicatrical ectropion, different tumors or it may occur as a result of surgical interferences concerning tumor removal.

4. Esthetic blepharoplasty

Many ageing people complain their eyebrows and tissues around eyes go shifting down under the gravity force forming “fat hernias” in periocular area. Eyeballs move down and inwards the orbits, and fat tissues move to the front forming “eyebags” in the lower eyelids area.

Different surgeries are performed depending on situation. It is possible to remove the excessive amount of skin, fat tissue, orbicular muscle of the eye. Operations are performed on upper as well as on lower eyelids, and very often on both eyes. It is very important for a patient to understand the purpose of his/her visit to a doctor. 5. Anophthalmos surgery and plastic surgery before ocular prosthetics

In some cases, eyeball has to be removed. This kind of operation is performed only in certain cases and decision on its performance is made by medical experts consilium.

Mostly performed operations are enucleation, which is the entire eyeball removal, or evisceration, when the scleral-muscular ring of the eyeball is maintained. Due to preservation of the outer shell and eye muscles, the latter operation has the advantage in the smallest cosmetic defect achieved with subsequent prosthetic eyeball, ensuring the maximum possible mobility of the supporting stump.

It is clear that the choice of an operation is determined by the presence of indications or their absence, as well as contraindications.
